One such condition is Median Arcuate Ligament Syndrome (MALS).As explained by Dr Jignesh Gandhi, Robotic & GI Surgeon, Mumbai, “MALS is a very rare condition, with an incidence of around 2 per 100,000 patients that occurs when a string of tissue presses down on the celiac artery and restricts blood supply to the organs in your abdomen, which leads to severe pain. MALS symptoms are frequently confused with those of common medical conditions such as gastritis, IBS, or even anxiety, causing a lot of patients to get the wrong diagnosis and continue to be in discomfort over longer time periods than they would if they had been diagnosed correctly the first time.”Because it is rare, it is often not the first thing doctors look for. And that is exactly why it gets missed.

The cost of delayed diagnosis

Living with unexplained pain changes daily life in quiet ways. Meals become stressful. Social plans shrink. Energy dips.Dr Gandhi adds, “Many patients who ultimately find out that they have MALS have been through a long series of treatments without relief before they discover what is wrong with them. Pain that is consistently triggered after eating, progressively worsens, and is associated with weight loss, nausea, or vomiting should not be ignored. These signs warrant deeper evaluation.”There is also a mental toll. Repeated misdiagnosis can make people doubt their own symptoms. That delay often stretches the recovery timeline.

Early diagnosis not only reduces suffering but also opens the door to effective, advanced treatments.

Treatment has evolved, but timing matters

The good news is that treatment has improved.Dr Gandhi explains, “Surgical decompression of the celiac artery remains the standard treatment, but it is technically challenging due to its proximity to major blood vessels like the aorta. Here, advancements in minimally invasive techniques are making a difference.”He further adds, “However, the development of surgical techniques, specifically Robotic-Assisted Surgery (RAS), has completely changed this. For example, the RAS systems such as da Vinci surgical system give surgeons incredible levels of control, 3-dimensional (3-D) high-definition viewing ability, and significantly more instrument dexterity than what is possible with human hands. These advances enable surgeons to perform much more precise and meticulous dissections around a very crucial area of a major blood vessel called the aorta, helping in the release of the fibres of the median arcuate ligament, resulting in more complete, safer surgeries, less blood loss, and faster recovery times for patients, even in the most difficult cases of MALS.”Early diagnosis makes this journey smoother. It shortens suffering and improves quality of life.

Why paying attention matters more than ever

The body rarely stays silent without reason. Pain that repeats, especially after eating, is worth noticing.As Dr Gandhi concludes, “In order to receive proper MALS therapy, early and accurate diagnosis of the condition is critical to an increase in quality of life. Patients suffering from such ongoing pain should not overlook it and rather consult their physicians for further investigation into this issue.”There is a simple takeaway here.
